**About the Role**

APM is proud to provide Regional Assessment Services (RAS) to Older Australians. With the commencement of Single Assessment in 2025, phase 1 of the new system will commence in July 2024 with the introduction of the Integrated Assessment Tool (IAT) and within this a new triage process will commence to improve paving the pathway for Older Australians through their My Aged Care journey.

As the Triage Team Leader you will provide exemplary leadership to a team of Senior Client Service Officers who undertake a phone based standardised triage questionnaire with older Australians to enable access to the care they need to remain living independently in their chosen community. You will also conduct regular triaging of clients to ensure contemporary practice and capability to support both clients in their journey and Assessors in the community.

You will be responsible for the day-to-day management, support and development of a team of remote client service officers, developing and maintaining effective relationships with internal and external stakeholders, monitoring and achieving performance, KPI and quality outcomes and building/maintaining a positive team culture.

**To be successful in this role you must hold a tertiary qualification in Allied Health or Nursing.**
