**What You Will Be Doing**

The Community Connectors Program (CCP) is an exciting new initiative modelled off the successful High Risk Accommodation Response (HRAR), a program that formerly worked to support vulnerable clients in high-risk settings during the peak of the COVID pandemic. This new program is looking to employ a part-time, fixed-term Lived Experience/Peer Worker who will offer flexible and targeted mobile services within priority accommodation settings, engaging proprietors and residents to promote public health initiatives, identify strengths and needs and connect residents to local health and social services. The Community Connectors Program will provide assertive outreach to clients who are residing in low-rise, high-risk accommodation across the Frankston LGA. You will be required to provide 1:1 peer support to people with the CCP to promote hope, understanding and shared learning.
The successful applicant will work within a multidisciplinary team to actively consult and seek the opinions of clients and empower clients to participate in self-advocacy.

**What You Need**
- Personal experience of mental health, AOD or homelessness challenges and recovery
- Recognition and understanding of own expereinces and ability to use this to support clients
- Established skills and commitment to working with people who have experienced trauma with co-occurring morbidities
- Sound communication and documentation skills.
- Victorian Driver’s licence and willingness to transport clients to appointments as required
