**This is temporary part time 16 hours per week maternity relief position until 4 February 2024 with the possibility of an extension at same/reduced hours.**

**What you'll be doing**

This position is responsible for providing specialist Social Work services to patients, clients, consumers and their family / carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position provides the above Social Work service to Renal Supportive Care clients at clinics in Wollongong, Shellharbour, Shoalhaven and Milton Hospital and in the community. The position works as part of a multidisciplinary team and provides clinical supervision to Level 1-2 Social Work staff and university students. The position provides advice and education to both Social Work and hospital staff on psychosocial issues in the Renal Medicine clinical area.

**All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (3 doses for Category A roles, 2 doses for Category B roles) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.**

**Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.**

Successful applicants to the role must provide certified documentation of vaccination history and pathology as part of their recruitment documentation. The applicant must be certified as compliant with Occupational Assessment, Screening and Vaccination Against Specified Infectious Diseases before employment can commence.
- Extensive post graduate Social Work experience in psychosocial assessment, care planning, individual and family counselling, grief and loss and bereavement counselling, and group work within a health setting.
- Extensive specialist psychosocial knowledge relating to people with a life threatening illness (such as a serious renal diagnosis) adjustment to diagnosis / disability, as well as loss and grief theory and practice, crisis theory and sudden death.
- Demonstrated knowledge, comprehensive training and a sound understanding of relevant legal processes such as guardianship. The ability to provide staff education as required.
- Demonstrated high level ability to communicate clearly and succinctly both verbally and in writing, and proven ability to work effectively and collaboratively within a multi-disciplinary team. Demonstrated competence in the use of electronic information systems.
- Demonstrated ability to initiate, lead, complete and share quality improvement activities and service evaluation processes with other staff and demonstrated capacity to provide clinical supervision.
- Current NSW Drivers Licence, with the ability to travel in accordance with position responsibilities.
