Tweed Mental Health Service is seeking a social worker to join The Homeless Health Outreach Team to address the clearly demonstrated need for services for homeless people with mental illness and/or alcohol and other drugs issues. Aiming to support people experiencing or at risk of homelessness to actively engage in healthcare and access mainstream services of their choice, including assisting people to access specialist health care and ensure they are supported in improving their health and wellbeing outcomes.
Providing services to people who are chronically homeless with complex needs and diverse value systems, and who do not access standard health services. While there is a strong focus on mental health and drug and alcohol issues, general physical health issues will be within the scope of this team. The team will work very closely with the non government services and other local and state government services that are involved in providing services to this clientele.

Ensuring delivery of effective, responsive and clinically appropriate services within the NSW Health Performance Framework.

**_If you have:_**
- Relevant qualification in Bachelor of Social Work and eligible for membership of The Australian Social Work Association prior to commencing. To be appointed as a Level 3 Health Professional evidence must be provided of three years' clinical experience and demonstrated experience of extensive specialist knowledge or a high level of broad generalist knowledge within the discipline as defined by the award definitions of a level 3 practitioner
- Skills working both in the mental health setting and the community
- Experience working across all age groups including adolescents and the elderly who may be from culturally diverse backgrounds and divers value systems
- Advanced holistic consumer care needs knowledge including the assessment, planning, implementation and evaluation process within a recovery and social work framework
