Specialising in palliative care, rehabilitation, older persons' mental health, dementia and aged care, HammondCare’s sub-acute health services are located at Prairiewood, Greenwich, Wahroonga and Mona Vale. **About the role**

The role of the Level 1/ 2 Social Worker is to work within the Mission and Mission in Action of HammondCare to ensure a high standard of Social Work service is provided to patients of the Hospital, along with appropriate support and advice to their carers. The Social Worker will participate as a member of the multi-disciplinary team to ensure a client-centered, evidence-based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and the Australian Association of Social Workers (AASW).

This role is based with Braeside hospital located at Prairiewood, and offers the opportunity to develop over time across a number of specialities through rotation.

The essence of this role:

- Provision of a holistic psychosocial perspective with effective and efficient social work service to clients, including appropriate support and advice to their carer and family
- Provision of emotional support, counselling and planning for clients, to facilitate their adjustment to major medical event, functional or cognitive change, a new diagnosis, or entering end of life care, whilst also preparing and supporting their carer/s or family members
- Participate as a member of the multi-disciplinary team in assessing the patients’ level of functional, care and support needs, collaboratively setting achievable goals with the client, and providing appropriate interventions to contribute

**About you**

**ESSENTIAL**:

- 1. Commitment to align and engage with HammondCare’s Mission in Action
- A tertiary qualification in Social Work, providing eligibility and membership to the Australian Association of Social Workers. Qualifications obtained overseas must provide AASW Overseas Assessment number
- Current working rights for Australia / valid Australian working visa
- Knowledge and or experience and skills in planning, implementation and evaluation of a range of psychosocial interventions. Demonstrated understanding of complex discharge planning for patients with a wide range of health conditions in hospitals is highly desirable
- Knowledge of relevant community services and referral systems
- Effective communication and interpersonal skills, using a variety of media - oral, written and computer
- Self-motivated and skilled in effective time management, prioritising competing work commitments and managing and responding to demanding situations
- Ability to interact constructively and collaboratively with the internal multidisciplinary team and external service providers
- Commitment to ongoing professional development, learning opportunities and professional supervision

**DESIREABLE**:

- Theoretical and practical understanding of issues pertinent to palliative care, rehabilitation or psychiatry settings
- Previous experience in providing Social Work in palliative care services
- Desirable experience in using NSW Health EMR - electronic medical record program
