**About HCS**:
HCS offers a wide range of support services to individuals living with disabilities in Brisbane, the Gold Coast, and Darwin. As a registered NDIS service provider, we are committed to providing tailored, flexible, and high-quality services that enhance our participants’ overall well-being and independence.

**Responsibilities of Lifestyle Support Worker**:

- Deliver tailored support services to individuals with disabilities, in accordance with their individual plans and goals.
- Assist participants in daily activities such as personal care, household tasks, community engagement, and social outings.
- Foster a supportive and inclusive environment that promotes independence, choice, and empowerment.
- Collaborate with the participant’s families, support networks, and healthcare professionals to ensure comprehensive care and coordination of services.
- Maintain accurate documentation and progress notes in accordance with NDIS guidelines and organizational policies.

**Requirements**:
To be considered for this role, applicants must meet the following requirements:

- NDIS Worker Screening Clearance.
- Completion of the NDIS Worker Orientation module.
- Police check conducted within the past 6 months.
- Valid First Aid and CPR certification.
- Blue Card (Working with Children Check) or willingness to obtain.
- Driver’s License and access to reliable transportation.
- Evidence of relevant qualifications in Disability Support, Individual Support, or a related field.
- Proof of Australian _work_ rights (current visa or Passport).
