**A little about the role**
2 x EFT fixed term positions (1 x maternity leave cover + 1 x fixed term until August 2023) have become available, suitable for experienced Community Health Dietitians. You will be working alongside Allied health and Medical staff to deliver best practice care to people within the DPV catchment areas of Hume and Whittlesea. Travel across sites will be required.
Your day-to-day responsibilities will include:
- Providing evidence based, client centered, goal directed assessment and care planning to a diverse caseload of clients
- Contributing to the multi-disciplinary team, delivering team-based care plans, case reviews and initiating referrals as appropriate
- Involvement in community health projects and activities
- Travel across sites maybe required.

**About you**
You will have:
- Qualifications in Nutrition and Dietetics and be eligible for the Accredited Practising Dietitian.
- Experience in community health settings. Working with Paediatric clients will be highly regarded
- Well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with client,
- Experience in planning, implementing and evaluating innovative, evidence based, clinical services
