Why this role matters
The Salvation Army Australia’s Homelessness Stream provides high-quality, person-centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.
About the role
We are seeking a values-driven and proactive Case Manager to provide case management to people who are homelessness or at risk of homelessness.
Reporting to the Support Services Team Leader, this is a Fixed-Term(until July 2025), full-time position based in Geelong,VIC. Salary and conditions are in accordance with the SCHADS Award Level 5.
You will successfully- Undertake in depth client assessment for intake and the provision of information and referral when needed.- Develop holistic individual case plans that encompass a range of strength-based and solutions-focused interventions in collaboration with clients to address their needs and goals.- Develop appropriate and realistic housing plan for all clients participating in the program to assist clients in making informed housing decisions.- Facilitate an Accommodation Living Skills Program to clients experiencing homelessness.- Provide reports (oral and written) to the Team leader or Manager as requested.
You will have- Social Work qualification or other relevant social tertiary qualifications and/or substantial experience.- A current and valid Victorian Employee Working with Children Check.- A current Victorian Driver licence.- Experience and resilience in working with complex clients presenting with challenging behaviours.- Ability to encourage and cooperate with others to achieve common goals.
