**About the role**
Here at EACH we are looking for an
**Outreach Worker** to join the newly created Community Connectors program which aims to improve health literacy, connections and engagement across a wide range of public and community health topics, including Covid-19 prevention and other topics as required.
- As Outreach Worker you will engage and work with residents of public housing, community housing, supported residential services, caravan parks and rooming houses in the LGA’s of Greater Dandenong, Casey, and Monash.
In this fast-paced role, no two days will be the same. Drawing on your experience with vulnerable community members you will Support clients and facilitate referral pathways and develop and deliver health promotion strategies targeted to the local community.

**Who we’re looking for**
In addition to a self-directed passion and drive to support vulnerable people within the community, you will have:

- Community outreach experience particularly with marginalized and hard to reach community members
- Experience working with people with complex needs (mental health and AOD concerns)
- Experience in housing/homelessness services
- Sound assessment and reporting writing skills
- Mandatory Qualification/s, Competencies and/or Licences_
- Relevant tertiary qualifications in health or an allied health discipline

**Who you’ll be working with**
The aim of the Community Connectors Program is to provide active linkages, employing people with lived experience and health promotion workers to link people at their doorstep to preventative and early intervention health and social care services.
