**About the Role**:
We are proud pioneers of 24-hour care, and we're searching for a talented Case Manager to support our HCP client program, including 24/7 clients. As a Case Manager, you'll be the advocate for our clients, ensuring tailored, holistic care that meets their individual needs. From comprehensive client assessments to developing personalized care plans, you'll play a pivotal role in monitoring and reviewing client wellbeing. Collaborating with stakeholders and acting as a brand ambassador for Prestige Inhome Care will be essential. You will build strong relationships with clients and work closely with our service coordination team to provide knowledgeable support and a dedicated care team.

**About You**:

- Previous case management experience
- Excellent assessment, documentation, and care plan implementation skills
- Experience working with people with complex care needs, including those with dementia-related behavioural aspects
- Exceptional communication, negotiation, and interpersonal skills
- Strong prioritisation, time management, and organisational abilities in a fast-paced environment
- Ability to manage client expectations and resolve concerns effectively and promptly
- Sound administrative and self-management skills, with a problem-solving mindset
- Proactive approach, capable of working independently and collaboratively
- Discretion and commitment to maintaining confidentiality
- Excellent verbal and written communication skills, with a client service focus
- Tertiary qualification in health and/or social work (preferred but not essential)
