**What you will be doing**:?
An exciting opportunity is available for a Social Worker to join the team at Blue Mountains Hospital on a permanent full time basis. The purpose of this position is to provide a comprehensive Social Work service to patients, carers and families to ensure optimal health outcomes consistent with NBMLHD and AASW policies, procedures, standards relevant legislation and the NSW Health Code of Conduct.

This role may be required to provide Social Work relief cover to other wards at both Blue Mountains and Springwood Hospital, as required by the Clinical Senior Social Worker.

**Selection Criteria to be Addressed**:

- Degree in Social Work with eligibility for membership of the Australian Association of Social Workers (AASW).
- Demonstrated understanding of psycho-social impact of unexpected hospitalisation due to illness, accident and trauma.
- Experience in working in acute hospital setting and/or a medical or physical rehabilitation setting.
- Demonstrated ability to complete psychosocial assessments and to formulate and implement appropriate discharge and care plan in a short time frame for patients in an inpatient setting.
- Proven ability to work as part of a multi-disciplinary team.
- Demonstrated high level of written and oral communication and interpersonal skills
- Current NSW Drivers Licence, with the ability to travel in accordance with position responsibilities.
