About the role

This primary responsibility of this role is the coordination of services by Aspire Community Services Workers to deliver quality NDIS services to people within the service. The Service Coordinator is to provide leadership, support, and supervision to residential workers. This role is responsible for building a rapport with participants to understand their situation and coordinate service delivery ensuring Aspire Community Services workers are working with participants to meet outcomes of each individual plan.

As the Service Coordinator you are responsible for overseeing the day-to-day operations of the Residential SIL (Supported Independent Living and STA - Respite) Houses. You will mentor, supervise, and lead a team of support workers providing care and support for participants with high and complex needs in a residential setting.

Duties & Responsibilities
- Provision of supervision, mentoring and performance reviews of the Team Leaders/ staff.
- Work with staff, participants, families and other stakeholders to understand and resolve concerns. Escalate to the Service Manager for advice and support as required.
- Audit schedules to be created and updated as part of the internal audit process
- Ensure that staff are rostered according to skills and best match for shift as well as participant.
- Shift Management
- Ensure to remain well informed on the requirements of being a registered service under the NDIS.
- Processing and Management of Incident Reporting, Injury Near Miss, Complaint reporting, and Work Health and Safety.
- Maintaining and assessing risks and coordinating risk management strategies.
- Monitoring and reviewing of service delivery as per the service agreement.
- Working with the Direct Support Staff and clients in each service area to collaboratively develop plans, tools and resources to be implemented.
- Work with assigned clients and families to develop and implement Person-Centred Support Plans aligning with NDIS goals and funding allocation.
- Maintain client data integrity to meet organisational and contractual compliance.
- Contribute as required to the reporting process.
- Develop and nurture strong relationships with all stakeholders to promote Aspire Community Services in a positive way within the community
- Utilise computer and software systems to ensure effective service delivery and communication
- Conduct, attend and contribute to all one on one and team meetings as directed

Essential Criteria

To be considered, you MUST have:

- An understanding of disability and community services sector
- Relevant qualifications and experience and competence in the human services field
- Minimum 2 years industry experience
- Comprehensive understanding of NDIS
- Computer skills in Microsoft Office packages
- Current Victorian Drivers Licence
- Current National Police check (valid for one year)
- Current Working with Children’s Check
- NDIS Worker Screening Check (mandatory)
- Commitment to working within the legislative framework and the organisations policies and procedures
- Fully Vaccinated
- Minimum two referees

**Job Types**: Full-time, Casual

Schedule:

- 8 hour shift
- Monday to Friday
- Morning shift

Licence/Certification:

- NDIS Worker Screening Check (preferred)
- Police Check less than a yea (preferred)
- Working with Children Check (preferred)
- First Aid Certification (preferred)

Work Authorisation:

- Australia (preferred)
