We have an exciting opportunity for a motivated and person centered Grade 2 Social Worker who is keen to develop their leadership skills and provide high quality care to patients within the Acute Rehabilitation Program.
The Acute Rehabilitation Program (ARP) is an interdisciplinary service that provides early rehabilitation in the acute setting at Frankston. The Social Worker in the Acute Rehabilitation Program is an integral part of the multidisciplinary team that assists patients in regaining their independence and confidence to support a timely discharge home where possible. The ARP Social Worker will contribute to the team by demonstrating a commitment to improving the health outcomes for patients.

- You will be supported and encouraged to develop your leadership skills and be involved in a new and exciting program

**What You Will Be Doing**

The Social Work Department is an innovative and dynamic team with a focus on the provision of high quality care. The Acute Rehabilitation Program (ARP) Social Worker is a key member of the Social Work team, and is also an integrated member of the ARP multidisciplinary team providing Social Work services to patients receiving rehabilitation on acute wards at Frankston Hospital.

**What You Need**

We anticipate you will have:

- You will need to hold a Bachelor Degree or Qualifying Masters in Social Work. Post graduate studies are highly regarded and may be eligible for a higher education allowance
- Victorian Drivers licence.
- You are passionate about providing patient centred care and working as part of a team.
