Join New Home Care and support older people to live well in the comfort of their own home. We are looking for an energetic and passionate case manager to join our team in assisting clients and their families to live their best life.
**About the role**
- Developing relationships with families and careers that acknowledge their needs.
- Provide case management support to meet client’s individual needs in accordance with the Aged Care Quality Standards.
- Develop and review comprehensive person-centered care plans. Conduct risk assessments in consultation with clients and relevant stakeholders.
- Liaise with internal and external services agencies to ensure safe implementation of care plan goals
- Investigate and respond to consumer enquiries, concerns, or complaints in a timely manner
- Assisting in reporting, auditing and continuous improvement activities.
- Implementation of the latest best practice and evidence-based case management approaches.
- Assist in new Home Care Packages sign-ups by providing appropriate information to prospective clients.
- Participate in project work as directed by the Home Care Manager

- Qualification in Cert IV Aging Support / Case Management / Allied Health or Nursing with relevant primary care experience.
- Demonstrated experience in caring for older people.
- Excellent problem-solving skills for complex clients and appropriate decisions making capacity.
- Assessment and care planning skills
- Excellent communication skills
- A team player
- IT skills - MS Office basic, intermediate level of skill across all packages or Office, Word and Excel
- Knowledge of Home Care Packages and Aged Care Quality Standards
- Fluent in Chinese or Korean is a plus.
