Company Description

EACH is the Lead Agency of the Therapeutic Alliance (TA), a new consortium of community based, not-for-profit services who provide services across the North Coast of NSW. The alliance will deliver the Primary Mental Health Program (PMHP) through a multidisciplinary, person centred, hub-and-spoke model of care.

As Clinical Care Coordinator you will be responsible for leading the day-to-day operations of the local hub, facilitating the care planning and review process for people accessing services to the Primary Mental Health programs, as well as collaborating with service providers.
Duties include:

- Monitoring and coaching individuals to achieve team outcomes, while also fostering a culture of continual quality improvement & consumer-led care.
- Managing the provision of the services from the hub in conjunction with consortium partners.
- Facilitating care planning, assessment, review, and care-integration processes for people accessing hub services.
- Monitoring processes for ensuring mental health needs escalation, including appropriate referral and transition to tertiary care when required.

**Qualifications**:
As the Clinical Care Coordinator, we are looking for someone with extensive experience working within a mental health setting supporting young people and families, and a commitment to delivering a person-centred approach. Knowledge of the mental health sector and its related policies, procedures, and regulations, as well as knowledge of services offered the community to support those with complex needs will be essential to succeed in this role.

To succeed in this role you will also have:

- Demonstrated experience in leading, mentoring and providing clinical support to other clinical staff within the team to achieve positive client outcomes.
- The ability to provide clinical service delivery leadership with capacity to assess needs and provide interventions across a range of life areas and needs.
- Strong computer skills, including proficiency with Microsoft Office and relevant software.
- Demonstrated ability to develop and maintain relationships with a range of government, community and corporate stakeholders to facilitate clear referral pathways.
- Mandatory Qualification/s, Competencies and/or Licences_
- Tertiary qualifications in psychology, social work, mental health nursing and/or other.
