**About You**

To meet demand for our services, we are seeking a self-motivated and passionate individual to provide Support Coordination or Psychosocial Recovery Coaching for our clients based in Perth metro area.

You must have relevant experience and qualifications for the Psychosocial Recovery Coaching role. This includes lived experience and/or experience working in mental health services, experience in case management, support coordination, and care coordination, as well as any relevant credentials in health, social work, disability, mental health, community services, or alcohol and other drugs.

You will require a good understanding of NDIS Coordination of Supports and a willingness to provide exceptional personalised assistance to our participants.

You will need to be comfortable working remotely, have your own reliable vehicle, a current driver’s licence and have excellent knowledge of the NDIS administration processes and be prepared to commit to ongoing learning.

**About the Role**

Multiple positions working with participants in Perth and the surrounding suburbs are available for Support Coordination and Psychosocial Recovery Coaching. Mid-March is the start date for these positions, with some flexibility to accommodate notice periods.
- Work from home and within the community primarily.
- Casual positions with a minimum commitment of 0.6 FTE
- Your minimum availability (3 days per week).
- The purpose of this position is to assist National Disability Insurance Scheme (NDIS) participants with psychosocial disability in gaining access to appropriate services and understanding their NDIS funding and options, as well as in formulating their desired goals.
- Using each individual plan as a guide, we assist participants in acquiring the necessary resources and services, while maintaining a strong and respectful relationship with them.

**Essential Criteria**

Hold or be willing to obtain:

- NDIS Worker Screening Check
- Current Working with Children's Check
- Current valid First Aid and CPR Certificate
- NDIS Worker Orientation Module Certificate
- COVID-19 Vaccination
- COVID-19 Training and Control Certificate
- WA Driver’s Licence

What you will need:

- Have excellent written and verbal communication skills
- Have a reliable insured vehicle
- Minimum 2 years’ experience within disability services or related case management in Mental Health services
- Proven experience working with people with disability and / or mental illness
- Demonstrated ability to network with local service providers, NDIA and participant support network
- Demonstrated experience in Support Coordination, Psychosocial Recovery Coaching, support planning, working within a person-centered framework
- Experience working within the NDIS environment
- Understanding of relevant privacy and confidentiality in relation to participants
- Comprehension of what is considered appropriate self-disclosure
- Experience with the NDIS Price Guide and flexibility within budgets, the NDIS legislation and rules including provisions relating to reasonable and necessary support and the role of the mainstream service system

**Salary**: $35.00 - $40.00 per hour
