**About the Complex Care Management Team**:
The 4 specialist services (Respiratory, Heart Failure, Diabetes, Generalist Care Management) of HIP Complex Care aim to assist people with chronic disease and aged and/or complex needs who frequently use hospitals or are risk of hospitalisation. Our intention is to assist these vulnerable people to remain living at home and to prevent unnecessary representation to the Emergency Department or admission to Hospital.

The HIP DCAS Dietitian is a member of the multidisciplinaryDCAS team and works alongside the Diabetes Nurse Educators and Endocrinologists to provide specialist Diabetes information and education for complex clients with diabetes and other chronic conditions.

**About the role**

To be successful in this role you will:

- be eligible for membership of Dietitians Australia and the Accredited Practicing Dietitian Program.
- have at least 2 years post graduate clinical specialty experience.
- have a Client Centred approach to service delivery,
- and well developed assessment, care plan development & problem-solving skills.
- Have the ability to problem solve in a variety of complex situations.
