**About the opportunity**:
As an Early Intervention Case Manager, you will provide holistic case management, information and advice to families and single adults over 26 years old to maintain or sustain housing. In this role, you will advocate on behalf of clients in relation to housing and other issues, as well as encourage & assist clients to access a range of services relevant to their specific needs.

You will also be required to outreach support to Wyndham residents who reside within the municipality or have links to Wyndham through network support, family friends’ employment or education.

**About you**:
You are looking to join an organisation that truly supports vulnerable Victorians and Tasmanians to improve their lives.
- A mandatory qualification in Human Services field e.g., Social, Work, Youth Work, Welfare studies OR where you can demonstrate relevant work experience and/or studies towards completing one of the above qualifications
- Current Victorian drivers’ licence
- Experience in direct service case management
- Experience with use of client data entry program SHIP, preferable but not essential
- Ability to meet the needs of service users adhering to the assessment and intake policies of the agency
- Ability to provide information and advice to clients in relation to their rights and responsibilities and encourage their full participation in decision-making.
