About the role

We are seeking a values-driven Freedom Pathways Support Worker who will nder direction, The Freedom Pathways Support Worker role provides support to community members to assist them to achieve set outcomes. The position will support the community members by providing referrals and connections to internal expressions and external community-based services.

You will successfully
- Community members accessing the service are provided with a welcoming and responsive initial contact and urgent and immediate needs are addressed
- In consultation with more senior worker/s/Line Manager appropriate preventative action is taken in relation to potential high-risk situations as needed
- Effective relationships are established with community members that encourage self-determination and participation and support them to improve their situation
- Information is provided to assist in assessing and addressing community members issues, needs and circumstances
- Support is provided to community members by offering referral pathways through to internal expressions, programs and external community-based programs including: Community dinners; Just Men; Family Fishing; God’s Sports Arena program
- Community members are followed-up and assisted to access support services
- Maintain effective working relationships, and support line management with volunteer engagement as needed
- Accurate, up to date community members records and data is maintained for all clients in accordance with TSA standards and procedures
- Service gaps are identified and recommendations for service delivery improvements are made through to Line Management for consideration
- Risks to clients, staff and TSA are identified and bought to the attention of relevant supervisor as soon as possible

This is a part-time (15hrs per week) fixed-term (12 months from hire) position located in Caloundra, QLD. Salary and conditions are in accordance with the SCHADS Level 3 award.

You will have
- Certificate IV qualification in social work, welfare, community development or related field with relevant experience
- Demonstrated relevant experience, knowledge and skills in a social service environment.
- Experience working with individuals from vulnerable and culturally diverse backgrounds
- A national police record check is required
- An international police record check is required, where applicable
- A Working with Children check is required
- Current First Aid Certification (or willingness to obtain one)
- A current driver’s licence
