**The Reconnect Mental Health Service makes early identification of emerging social, emotional and mental health issues in young people. We assist young people to improve their mental health and achieve their goals.**

**The Role**

The Reconnect Mental Health Service is an outreach, early intervention service supporting young people aged between 12 - 18 years and their families, where there is a risk of homelessness and emerging mental health concerns for the young person. The service is based at Centacare’s Port Adelaide office.

The Mental Health Worker will work closely with the young person, their family, the school and community to facilitate the young person’s integration into the family and education.

This is a fixed term opportunity until 30 June 2026.

**To be successful in this role you will need**:

- A minimum of a Certificate 4 in Mental Health or equivalent.
- Experience working with young people with emerging mental health concerns.
- Valid Department of Human Services (DHS) Working with Children Check (previously known as ‘Child-Related Employment Screening Clearance’).

**Desirable**
- Degree in relevant Human Services discipline.
- Experience using Penelope electronic client management system and DSS Data Exchange.
- Experience in facilitating groups for young people.
