About the role

The Program Coordinator, Individualised and Community Programs, manages a team of frontline support staff to deliver high quality, individualised one-on-one services for clients with a disability within their home and the community. This role is part of CPA Lifestyles services team

You will be responsible for:
Service Delivery and Stakeholder relationships
- Facilitating the intake process and ongoing service delivery for clients
- Collating client information and assess funding requirements and support needs to determine suitability for program participation
- Conducting client home visits and appointments and applicable assessments (including WHS) to facilitate the completion of the intake and assessment for service processes
- Maintaining up-to-date records and documentation to provide continuity in information sharing and to ensure compliance with legislative/regulatory and organisational policy requirements
- Monitoring client feedback to ensure high levels of client satisfaction are maintained
- Key contact for responding to requests for service from internal service managers, and decision making relating to ability to service based upon client needs, environment and available DSP resources
- Key contact for external stakeholders on new service delivery or changes to service delivery from clients and support coordinators.
- Developing and maintaining relationships with client’s families and support networks to share information, resolve issues and concerns and to ensure quality client outcomes are achieved.. Develop and maintain relationships with key internal stakeholders to promote program and service opportunities within CPA’s existing client base.

Resource Coordination
- Identifying and assessing employee availability, capability and suitability to effectively match resources to client needs and business requirements
- Overseeing/coordinating employee rosters to ensure client requirements are met and customer satisfaction is maintained
- Coordinating buddy shifts to ensure new employees are equipped and work-ready to support clients individualised needs
- Providing on-call support (as required) to ensure the delivery of a quality and responsive out-of-hours service and managing emergencies, enquiries and rostering issues

People Management
- Providing direction, supervision and support to ensure the team attains the required level of skills and capabilities to deliver quality services for clients.
- Coordinating the induction and onboarding of new employees to ensure compliance and alignment of skills and capabilities with the client and the organisation’s requirements
- Coordinating the completion of the employee’s essential initial and ongoing training and skill development to ensure compliance and alignment of workforce capabilities with business and client requirements.

We are seeking a self motivated individual who:

- Ideally has come from the disability or like industry
- Demonstrated experience dealing with stakeholders
- Minimum of two year’s demonstrated experience in a demanding administration role
- Ability to deal with conflicting priorities and meet strict deadlines
- Intermediate MS Office skills
- High energy levels, resilient and with a “can do” attitude
- Triple vaccinated against COVID-19
