**Your Opportunity**

The Aftercare Worker will support young people to transition from the Walawaani program to the community, working to sustain progress made at Walawaani and minimise relapse of Alcohol and Other Drugs, as well as engaging young people to address their physical, social and emotional needs.

**Your key responsibilities will include**:

- Assess the placement needs of young persons including ongoing treatment, accommodation, employment, training, social and emotional needs.
- Ensure aftercare processes are implemented and completed for each client
- Assist in the placement needs of each client at the service and post-completion including accommodation, training, education, and other special support services
- Assist and encourage clients to develop skills necessary to achieve their set goals and case management plans
- Participate in case conferencing for each young person with relevant key stakeholders, client, and their family member
- Complete a range of internal and external reports relating to clients and the program
- Maintain regular contact with young people in the aftercare program and with key stakeholders identified in their individual case plan.
- Develop partnerships with service providers across a spectrum of service types to be able to link young people with services in the community.

**Requirements for Success**
- Certificate IV or equivalent or higher qualification in the field of youth work, Alcohol and Other Drugs, Mental Health or relevant health/human services field.
- Experience working with young people with challenging behaviours, substance abuse and/or mental health
- Knowledge and understanding of the non-government alcohol and other drugs, mental health and youth sectors
- Demonstrated experience of case management
- Demonstrated experience working and collaborating effectively with others
- High level of numeracy, written and verbal communication skills and ability to keep up to date client records
- Evidence of full vaccination against Covid 19
- Current unrestricted NSW Driver’s license
