We are seeking for experienced HCP Case Coordinator to start with us. We are a small team in Springwood office and happy to share that we are growing each year.

**ABOUT THE ROLE**

Become a part of a dedicated and passionate team, work alongside with Case manager, other Case Coordinators and Rostering Team to ensure effective delivery of services. Your days will be filled with wonderful experiences as you meet with our clients, get to know them and their preferences, coordinate their care and support needs, achieving the very best practice care and support for them.

**Responsibilities**:

- Experience working in the care sector as a Care Coordinator or in a role related to the Home Care Package
- Strong capability to effectively coordinate clients care and support services
- Calm and Caring with excellence in client customer service
- Exceptional organisation skills
- Able to commit to continuous improvement
- Ability to work independently and also as a team ( team player)
- Demonstrated Empathy, Understanding, and Experience of working with vulnerable people living with Dementia.

**About You**:

- A can-do attitude and genuine interest in helping other
- Experience with challenging behaviours
- Practical understanding of Hygiene assistance and commitment of care
- Reliable and emotional stability
- Strong communication and organisation skills
- understanding of confidentiality
- Attention to detail and problem-solving skills

**Qualifications/Experiences**
- Previous care coordination experience
- Relevant qualifications or experience in leading direct care staff.
- Computer proficiency
- must have organizational and time management skills
- Professionalism
- Able to handle stress
- National Police Check

Your work hours:

- Monday to Friday
- 0830am - 0430pm

**Job Type**: Permanent Part-Time

Pay: Starting from $31 per hour

Expected hours: 37.5 per week

Work Authorisation:

- Australia (required)
