**Primary Responsibilities**
- Support Workers are responsible for assisting in the delivery of quality programs for MEGA TAG participants in accordance with the identified requirements of the PCP
- The Individual Lifestyle Arrangements program provides high quality community-based supports to people of all ages with a disability. The program aims to enrich people’s quality of life through a Person-Centred Active Support approach which promotes people’s:

- Dignity and respect in all aspects of their lives.
- Skills that are developed and utilised in a way which achieves their identified goals
- Participation in community activities.
- Choice in all aspects of decision-making which affects them.
- Relationship building with other people.

**Duties**
- Facilitate and encourage participation through the use of effective and augmented communication and interaction strategies.
- Participate in Staff induction
- Coordinate with participants, families and carers as required.
- Assist Participants to develop and monitor an individual plan that reflects their needs based on the fundamentals of Person Centred/Essential Lifestyle Planning
- Take part in Staff orientation
- Undertake all rostered or other duties as directed in a manner ensuring satisfactory service delivery according to the Disability Services Standard and Organisational Policies and Procedures
- Actively participate in program planning, training sessions and team building conducted by Management
- Implement and monitor Behaviour Management Strategies as required.
- Exercise knowledge/skills to assist in the co-ordination and delivery of programs in a manner that ensures best practice processes are followed.
- Provide support and assistance in the provision of personal care to all Participants as required.
- Actively participate in annual performance appraisals conducted by Management.

**Advocacy**
- Inform and support Participants in promoting their rights and responsibilities
- Uphold the rights, independence and dignity of all Participants, ensuring confidentiality and their choices are recognized at all times.
- Advocate on Participants behalf as required

**Administration**

Complete documentation not limited to but, including
- Incident and hazard reporting
- Communication diaries
- Medication sheets

**Key result areas**

Key result areas (KRAs) will be identified in consultation with the Operations Manager as part of MEGA TAG’ Performance Appraisal and Development process. These KRAs will be regularly reviewed and adapted as required.

**Qualifications / Experience / Knowledge**
- Possess a fundamental understanding of the Victorian Disability Standards, the Disability Act 2006 and their underlying principles, incorporating them into daily work practices and values.
- Current Victorian Driver’s license.
- Proven ability to work under broad direction.
- Fit to operate screen-based equipment.

**Knowledge & experience**:

- A clear understanding of Goal and Objective setting in relation to identified Participant needs.
- Experience in dealing and communicating with people with an intellectual disability.
- Experience in identifying, developing and evaluating communication matters.

**Skills**:

- Knowledge of general Policies
- Awareness of Practices and Procedures, particularly as they relate to communication.
- Good problem-solving skills.
- Knowledge of OHS Act, Equal opportunity Act,
- Proficient in the use of a range of computer programs i.e.: Microsoft Word & Excel.
- Good understanding of the operations and service agreements of MEGA TAG.
- Demonstrated knowledge of the Disability Services Award Part 1 MEGA TAG Enterprise agreement).
- Sound interpersonal skills with the ability to communicate with management, employees, and external Participants/customers.
