**About us**:

**The Opportunity**:
The Pathways to Good Health initiative is funded by the Victorian Government to deliver health screening, assessment, referrals and health planning to all Victorian children 0-17 years entering statutory care. Ballarat Community Health (BCH) has been funded to deliver this program across the Central Highlands Area for the next three years. BCH will extend our multi-disciplinary team comprising Paediatrician, GP, First Nations Navigator, Paediatric Nurse Coordinator, and Allied Health clinicians and run regular clinics, providing health checks and comprehensive health assessments. Our success will be in our effective collaboration with Area Child Protection, BADAC, Grampians Health, and family services organisations.
We’re looking for an experienced Nurse Coordinator to lead this exciting new program.
The Nurse Coordinator is a key leadership position within the Child and Youth Program area and providers oversite to the coordination and delivery of the Pathways to Good Health Program. This role will be responsible for managing the multi-disciplinary team, our key stakeholder relationships, maintaining efficient program systems, and ensuring performance targets are met. The Nurse Coordinator will also have a clinical role - triaging clients and ensuring the development of health summaries or care plans for children with complex health needs. The Nurse Coordinator will also work with a project team initially to establish The Pathways to Good Health Program.

**Responsibilities include**:
- Establish and support a multi-disciplinary team of clinicians to provide health assessments and referrals
- Ensure effective relationships with key stakeholders including Area Child Protection and Aboriginal Children in Aboriginal Care (ACAC)
- Provide triage, health assessments and referrals as required
- Ensure multi-disciplinary clinic meetings are held and appropriate documentation maintained
- Ensure the timely presentation of data, reports and the achievement of performance measures
- Provide clinical supervision as required
- Ensure the provision of culturally safe care
- Ensure the efficient running of clinic days and associated billing.
