Overview

Applications are sought from experienced and motivated Registered Nurses to join Echuca Regional Health as a Post-Acute Care Coordinator, supporting seamless transitions from hospital to home through coordinated, patient-centered care.

ERH services the Shire of Campaspe and the cross-border Murray River Council. Echuca Moama and townships are located directly across the state border from one another, separated by the Murray River. ERH provides a comprehensive range of acute, medical, surgical and sub-acute services plus residential aged care and a broad range of community healthcare services. Key responsibilities
  • Coordinate care by negotiating, purchasing, and monitoring community-based supports from contracted provider
  • Develop flexible and tailored care packages
  • Actively participate in mandatory meetings as required and requested
  • Maintain core clinical competence specific to the area of clinical practice
Qualifications/experience required
  • Current AHPRA as a Registered Nurse
  • Comprehensive knowledge of the discharge planning processes
  • Strong leadership and collaboration skills.
