Make an Impact as a Client Liaison Coordinator at MSWA
At MSWA, we're committed to providing tailored care and support for individuals living with neurological conditions, such as MS, Parkinson's, Huntington's, Motor Neuron Disease, Acquired Brain Injury, and Stroke. We are seeking a compassionate and skilled Client Liaison Coordinator to join our team and support our Home Care Package (HCP) clients in making the most of their care journey.

In this hands-on role, you will:

  • Build Strong Relationships: Work closely with clients and their families to assess their needs, develop personalised care plans, and guide them through their Home Care Package journey.
  • Coordinate Quality Care: Ensure the delivery of high-quality, person-centred services, while monitoring and adjusting care plans based on clients' evolving needs.
  • Manage Client Budgets: Oversee and manage client budgets, ensuring that funds are allocated effectively to meet care needs.
  • Collaborate with the Team: Partner with internal teams, including Allied Health and Nursing, to provide integrated and seamless care to clients.
  • Ensure Compliance: Stay up to date with legislation and industry standards to ensure all services meet required guidelines and regulations.
  • Make a Positive Impact: Be a key player in improving the quality of life for those in our care by providing reliable and empathetic support.

You'll be an essential member of a passionate, supportive team, collaborating with colleagues to ensure that our clients receive the highest standard of care. This is a dynamic role that will provide you with the opportunity to build meaningful relationships and contribute to the wellbeing of individuals living with neurological conditions.

A bit about you…
You have experience in aged care services, with a solid understanding of Home Care Packages (HCP) and funding. Your communication skills are excellent, and you're passionate about making a difference in people's lives. You're organised, adaptable, and have the ability to manage multiple priorities in a fast-paced environment.

To thrive in this role, you must have:

  • Experience in Aged Care Services, ideally in a similar role.
  • Knowledge of Home Care Packages (HCP) and funding.
  • Strong interpersonal and communication skills.
  • A genuine passion for helping others.
  • A Current Drivers Licence.
  • Strong time management and organisational skills.
  • NDIS Workers Screening Clearance (or willingness to obtain).
  • Current First Aid & CPR (we can help you with this if not).
