The Role

As an Aboriginal Health Worker/Practitioner, Primary Care, you will work as a key member of our multidisciplinary health centre team to provide flexible, holistic, and culturally sensitive health care services to Aboriginal and/or Torres Strait Islander people and apply key strategies for face-to-face patient care.

There is a focus on prevention and management of chronic disease for Aboriginal clients and supporting them on their journey throughout custody back into the community, particularly those with chronic or complex health conditions.

You will work collaboratively with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations, and other relevant health and community groups to ensure the delivery of seamless, timely and effective health care to Aboriginal clients.

Through the transition, there is an expectation that clinical activities will be included and applied to daily operations as per scope allowances.

This position also allows direct appointment of existing Aboriginal Health Practitioners into the role.
