Adolescent Family Services Practitioner – Uniting Vic.Tas, Blackburn, VIC.

Join to apply for the Adolescent Family Services Practitioner role at Uniting Vic.Tas. This is a permanent full-time position focused on delivering outreach-based assessments, mediation, case management and tailored interventions for young people aged 12-17 and their families or carers. The program aims to prevent statutory intervention and homelessness by addressing family conflict and relationship breakdowns, offering a range of interventions from early support to intensive services.

Responsibilities
  • Provide outreach-based assessments, mediation, and interventions to support young people and families, helping resolve issues and keep young people in the family home.
  • Develop Child and Family Action Plans addressing family conflict, parenting challenges, and high-risk behaviours.
  • Collaborate with schools, community services and stakeholders, making referrals for resources on family violence, mental health and education.
  • Ensure compliance with child safety standards, complete risk assessments and maintain accurate documentation; participate in regular team meetings and professional development.
  • Work flexible hours as needed to meet the needs of families and young people, with sensitivity to diverse backgrounds including Aboriginal, Torres Strait Islander, CALD and LGBTQIA+ communities.

Requirements & Skills
  • Bachelor of Social Work, Youth Work, or relevant tertiary qualification with practical experience in case management.
  • Ability to connect with young people and families, build trust, and engage effectively.
  • Experience in conducting complex assessments, preparing detailed reports, and developing actionable plans with clients.
  • Awareness and knowledge of working with diverse groups, ensuring cultural safety (Aboriginal and Torres Strait Islander, CALD, LGBTQIA+).
  • Background in working with or for child protection services.
  • Strong interpersonal, time management, and organizational skills to manage caseloads.
