Uniting AgeWell's Grampians Home Care program supports elderly clients to live safely and independently at home for as long as possible. We offer comprehensive ageing support and services, with local employment opportunities throughout the Grampians region, including Ballarat and surrounding communities.

We are currently looking to grow our team with passionate and caring individuals in our Home Care Worker role.

Key Responsibilities

Our Home Care Worker roles involve traveling between clients' homes to provide tailored support and care that meets their individual needs.

Responsibilities may include:

  1. Providing personal care, including showering, grooming, and mobility assistance
  2. Supporting clients with domestic tasks such as meal preparation and household chores
  3. Monitoring clients' physical, emotional, and behavioural well-being, and reporting concerns
  4. Providing social support and transport for appointments or activities
  5. Respecting clients' rights, choices, and cultural and spiritual needs

What We're Looking For

We're looking for individuals with a passion for supporting people to live independently in their own homes. A caring, empathetic nature is essential, along with a commitment to building strong relationships with clients and their families.

Successful candidates must also have:

  1. Reliable transport and the ability to travel between clients' homes.
  2. Excellent organisational skills, with a focus on maintaining safe, clean, and supportive environments for clients.
  3. A strong understanding of the person-centred approach and community care, with dedication to delivering high standards for clients and families.
  4. The ability to thrive in a collaborative team environment while valuing one-on-one client connections.
  5. A commitment to our organisational values and the BEST philosophy—Believe, Excel, Support, and Trust.

Mandatory Requirements

  1. Qualification: A Nationally recognised Certificate III/IV in Individual Support or equivalent (e.g., Diploma or Bachelor of Nursing in progress).
  2. Driver's Licence and Vehicle: A valid Australian licence and a roadworthy vehicle with comprehensive insurance.
  3. First Aid & CPR: Current certifications or willingness to obtain.
  4. Working with Children Check (Employee)
  5. National Criminal Police Record Check or NDIS Worker Screening Check
  6. Annual Vaccinations: As per organisational policy.
  7. Australian Work Rights
