Pharmacist Senior

Fixed term temporary full time opportunity to deliver a high level multifaceted clinical pharmacy services and leadership to in patients within internal medical teams at the RBWH. Contribute to practice and service development, innovation and activities that improve medicines management for patients accessing Pharmacy services, and that are aligned to organisational goals, professional, legislative and ethical standards.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

• As a general specialist demonstrate and deliver a high level of knowledge in clinical pharmacy and apply professional judgement to problems of significant scope and complexity, incorporating safe transition of care across different sectors.
• Communicate effectively with inpatient multidisciplinary teams on the opportunities for medication optimisation, and the plan for patient's medication as an inpatient, and as they cross healthcare settings.
• Provide advice and assistance to multi-disciplinary health professional staff about pharmacy services, policies and procedures, and promote quality use of medicines, medication safety principles, and the role of the pharmacist within the hospital
