About The Role

The Residential Aged Care In-Reach (RAC-I) Service is part of the Hospital in the Home (HITH) team, delivering compassionate, integrated care that helps vulnerable aged care residents avoid unnecessary hospital presentations.

As a Physiotherapist in this dynamic service, you'll be based in Maitland, supporting aged care facilities across the region through a combination of telehealth and face-to-face care.

No two days are the same in this role. You'll collaborate with multidisciplinary teams, liaise with acute hospitals and community services, and provide evidence-based, patient-centred care to residents and their families.

You'll be actively involved in clinical decision-making, crisis response, advocacy, and supporting patients through complex care journeys.

You'll be based in Maitland and work across the Greater Newcastle and Lower Hunter region. You are a capable, compassionate Physiotherapist with:

Requirements
  • Physiotherapist with current Authority to Practice with AHPRA
  • Eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve driving long distances.
