The Nepean Blue Mountains Local Health District Drug and Alcohol Service offers free, non-judgemental services and treatments for drug and/or alcohol use, including information and support for families. Our service offers specialist drug and alcohol doctors, nurses and allied health staff who respect the client's right to choose from a range of treatments offered with our services underpinned by a philosophy of harm reduction.
Join the Assertive Community Management team and be at the forefront of transformative care for individuals navigating drug and alcohol dependency. As a clinician, you'll deliver direct, frontline services to patients facing complex physical, mental health, and social challenges. Working within a medically led multidisciplinary team, you'll conduct comprehensive assessments, co-design care plans, and provide assertive follow-up, including home visits, to support recovery and long-term wellbeing. You'll help shape clinical procedures, strengthen referral pathways across community services, and contribute to team operations and reviews.

What You Will Bring To The Role
- Tertiary qualifications in Social Work or Occupational Therapy or Rehabilitation Counsellor. Current membership (or eligibility) with the Australian Association Social Workers (AASW) or AHPRA registration as an Occupational Therapist.
- Demonstrated clinical skills in assessment, planning and evaluation of psychosocial interventions with adult patients including exercising independent professional judgement on routine matters.
- Demonstrated experience with using advanced reasoning skills when undertaking a patient's assessment of cognitive capacity and their complex issues, report writing, developing and reviewing care plans, delivering case management activities and psychosocial interventions.
- Demonstrated experience and commitment to professional and ethical boundaries in consultation with a medically led multi-disciplinary treatment team.
- Knowledge and understanding of the social and emotional needs of patients with drug and alcohol issues including clinical response and management of trauma, grief and loss, domestic and family violence and child protection.
- Knowledge of and commitment to working and partnering with Aboriginal patients, families and communities as well as improving the health outcomes for socially disadvantaged groups of patients within the local community.
- Current Driver's License and willingness to use for work purpose travel.
