About Aligned Community Care

As a full-service NDIS provider, we offer Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Community Support, and Nursing Services. Our mission is to deliver high-quality, person-centred care that promotes independence, dignity, and well-being.

Your Role

As a Mental Health Disability Support Worker, you'll provide vital daily assistance and meaningful engagement, including:

Supporting personal care and independent living skills

Managing behaviours of concern using trauma-informed approaches

Assisting with public transport, job placement, and community participation

Collaborating with families, allied health professionals, and stakeholders

Promoting social inclusion and advocating for client rights

Who We're Looking For

You are a motivated, compassionate, and experienced support worker who:

Has at least 1 year of experience supporting people with mental health challenges or psychosocial disabilities

Holds (or is working towards) a Certificate III or IV in Community Services, Individual Support, or similar

Understands and applies trauma-informed and person-centred practices

Has experience with continence care and behaviour support plans

Demonstrates excellent communication, documentation, and teamwork skills
