Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north.

Services are provided through our five main campuses:

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Centre, and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.

The Role:

Northern Health are seeking applications from a motivated and suitably experienced Registered Nurses – to join the Community Nursing Post Acute Care team within the Hospital Without Walls division.

This position is Fixed term - Part Time - Maternity Leave Cover until 4 July 2026

The Northern Post Acute Care (NPAC) service works to ensure a smooth transition from hospital to home for patients with complex needs, who require more intensive support while they recuperate from their illness.

The Community Nurse will provide a pivotal role in supporting patients post hospital discharge, providing short-term nursing care for identified needs. The Nurse will provide clinical nursing for Post Acute Care and CHSP (Commonwealth Home Support Program) patients in their home. As a Community Nurse you will work with the NPAC Team Leader, other Care Coordinators and the PAC Community Nurses supported by a small team of administration staff – to support and coordinate the care needs for patients being discharged from an inpatient setting and requiring assistance to manage safely at home in the community.

To be successful in this role you will have:

  • Specialist clinical experience and knowledge in the management of wounds including knowledge of products, techniques and services available
  • Knowledge of diabetes and continence management
  • Experience in comprehensive initial needs assessment of client, identifying appropriate care options and making appropriate referrals as required.
  • Ability to apply relevant service criteria and guidelines to incoming referrals and to direct to the most appropriate service for the patient.
  • Demonstrated high level IT systems capability or ability to learn new IT systems.
