YES Service (YESS) is an independent registered National Disability Insurance Service (NDIS) provider specialising in the delivery of Support Coordination services.

YESS works closely with NDIS participants with a wide range of disabilities and psychosocial needs. We assist people to recognise their value, strengths, and goals. We place a high emphasis upon helping participants get the most out of their NDIS experience while making it easy for them to maintain or improve their health, wellbeing, social participation, and independence, as they grow their meaningful relationships.

ABOUT THE OPPORTUNITY

Due to our continued growth in Queensland, we are looking to recruit a NDIS Support Coordinator to assist our clients who require services provided by their NDIS plans. You will work with clients in Brisbane's Southern Suburbs and towards the Gold Coast, from your home office. This is a full-time position.

You will need extensive knowledge of the NDIS to deliver timely and effective support coordination services to a caseload of NDIS participants, ensuring they receive suitable options and are connected in a way that supports their goals. Your responsibilities will include:

  1. Providing Support Coordination services to NDIS Participants both independently and as part of a team.
  2. Assisting clients, families, and carers to connect with mainstream and disability services.
  3. Supporting during times of crisis with prioritisation skills.
  4. Accurately recording progress, outcomes, and actions, including timely reports to the NDIS.
  5. Responding to new support coordination enquiries.
  6. Working with new participants to establish and implement a Support Coordination plan focused on their goals.
  7. Building a reliable resource base of service providers in the local area.

ABOUT YOU

To succeed in this role, you should have:

  1. Strong applied knowledge of the NDIS and its service sector.
  2. High emotional intelligence.
  3. Excellent administration and record-keeping skills.
  4. Exceptional time management to meet multiple deadlines.
  5. Ability to manage a caseload in a billable hours environment.
  6. Tertiary qualifications in disability or community services, or substantial NDIS experience.
  7. Effective communication skills, both written and verbal.
  8. High computer literacy.
  9. Ability to work autonomously from a home office within a team environment.
  10. Self-starter with problem-solving skills.
  11. Current Driver's License and ability to travel to client sites.
  12. NDIS clearance, Working with Children check, National Police Check.
  13. Completed NDIS Workers Orientation Module.
  14. 100 Points of ID, Demonstrated right to work in Australia.
