HammondCare at Home is currently looking for an experienced Clinical Partner to support the team in the Illawarra and Shoalhaven area. The role delivers high-quality, person-centered clinical care in clients\' homes and works in partnership with clients and key stakeholders to address goals, needs and preferences.

Essence of the role

The Clinical Partner works within HammondCare's Mission in Action to provide evidence-based, best-practice clinical care, assess clinical care needs, develop and review care plans, and deliver excellent clinical care in the community. This role collaborates with the care management team to ensure optimal outcomes and provides guidance to care staff to ensure safe, continuous delivery of high-quality clinical care.

This position requires commitment to engage with HammondCare\'s Christian Mission, Motivation and Mission in Action.

Responsibilities
  • Assess clinical care needs and develop, review and update care plans.
  • Deliver evidence-based, person-centred clinical care in clients\' homes.
  • Collaborate with the care management team to achieve the best client outcomes in the community setting.
  • Provide guidance to care staff to ensure safe and continuous delivery of high-quality care.
Qualifications
  • Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
  • Passion for and experience in client-focused service delivery in aged and community/home care settings.
  • Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards.
  • Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care.
  • Ability to work effectively in a multidisciplinary team and independently across multiple sites.
  • A current driver's licence and ability to travel between HammondCare\'s sites and services.
  • Excellent administrative and computer skills, including digital health records and care coordination tools.
  • Commitment to professional development and a patient, empathetic approach to care.

Desirable: Nursing experience in aged care and community care. Ability to identify and implement innovations in care delivery and service improvement.
