Overview

Clinical Care Coordinator - Aged Care - Coppin Centre, Melbourne, VIC

Join the Royal Freemasons LTD team as a Clinical Care Coordinator at Coppin Centre Residential Aged Care Facility in Melbourne. This role involves delivering high quality clinical care to residents and coordinating care with the care team and health professionals.

Key Responsibilities
  • Undertake, or supervise others to undertake, clinical care for individual residents as required, including complex wound care, pain management, behaviour management, end-of-life care and other clinical procedures as necessary.
  • Liaise with Residential care staff, medical, allied health, hospital and other specialist health practitioners to ensure residents receive high level clinical care.

Qualifications, Knowledge and Experience
  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) via AHPRA
  • Recent experience in caring for high care and dementia residents
  • Knowledge of wound care, palliative care, behaviour management, clinical assessments, care plans
Other Requirements
  • Mandatory Police Clearance or NDIS Worker Screening Clearance (or both) to undertake this role in the Aged Care sector
  • Vaccination: COVID-19 (three doses) and Flu vaccine are encouraged; evidence of vaccination may be required prior to commencement
