At Access Telehealth, we're Australia's leading provider of telehealth supported Aged Care. Our mission is to improve the health and well-being of older Australians by combining compassionate care with innovative healthcare solutions. We're expanding and are looking for a dedicated Enrolled Nurse to join our Access Aged Care team within the eastern suburbs/hills of Perth.

What You'll Do
  • Provide clinical management and coordination across residential aged care facilities
  • Conduct regular resident reviews and support person-centred care plans
  • Work closely with a multi-disciplinary team, supported by a Nursing Regional Manager and admin staff
  • Build strong relationships with residents, families and facility partners
  • Use your clinical knowledge and IT skills to ensure quality outcomes
What You'll Bring
  • Diploma of Nursing and AHPRA registration
  • A minimum of 18 months Nursing experience (leadership experience highly regarded)
  • Strong organisational, problem-solving and communication skills
  • Confidence with IT and documenting resident needs
  • A driver's licence and reliable vehicle
