
Service Delivery Coordinor– Children and Youth

All Family Nursing Care are a nursing and disability support provider serving Brisbane, Queensland.
We provide all aspects of mental health support, drug and alcohol management and community nursing. We also cater for participants of the NDIS scheme in their various homes. We ensure to empower individuals to be as independent as possible. That way, they can have more choices and control over their lives.
As part of our commitment to supporting the most vulnerable in our communities, we now provide specialised services for children and young people under department of child services, particularly those who are at risk, vulnerable, or in out-of-home care.
The role is based On-site in Ipswich and provides services across Brisbane.
Who we are seeking:
- Ideally you will have experience and passion for working with high needs vulnerable and at risk children.
- You will be passionate about supporting people to live their best lives.
- You are a caring and capable individual who enjoys working with clients to achieve planned outcomes and meaningful results.
- You are a brilliant communicator who can communicate with children with complex needs as well keeping all internal and external stakeholders informed.
- You like to lead by example and have systems in place to support workers to provide quality services.
- You are an excellent multitasker with exceptional time management skills.
- Your attention to detail means that your reporting deadlines are never missed, incidents are recorded and close out, and your clients files are accurate and up to date.
What we are offering:
A competitive salary of $70-75K is available for the right candidate. Pay will be in accordance with the SCHADS award.

The successful candidate will meet the following criteria:
- Previous experience working with vulnerable and at risk children in a Senior or supervisory role.
- Ability to lead, manage, train and mentor team members.
- Ability to assist with planning and coordinating client care.
- Excellent interpersonal and communication skills both written and verbal skills and with both internal and external stakeholders.
- Ability to work effectively, respectfully and collaboratively in a team environment.
- Demonstrated time management skills and ability to multi-skill work duties to maximise productivity.
- A demonstrated continuous improvement approach
- Current disability worker screening check and Blue Card
- A current, unrestricted, Queensland Drivers Licence
- A National Police Certificate
