Overview

Care Worker required for TRANSPAC program in the Northern Beaches region. We are seeking a Cert III qualified person with at least 6 months experience in a similar role to join the Silverchain team. Work across Manly, Newport, Avalon, Forrestville, Seaforth and surrounding areas for TRANSPAC clients. TRANSPAC is a short-term restorative care program supporting clients to transition home after long term hospitalisation.

Responsibilities
  • Provide personal care services to aged care clients (e.g., getting clients out of bed, showered and dressed).
  • Assist clients with medications and mobility goals; support therapy goals as part of rehabilitation.
  • Provide complex care for clients with high care needs (such as dementia) and offer social and emotional support.
  • Transport clients to lifestyle activities or appointments and support family carers to take breaks.
  • Deliver home help services as a component of your roster, including light housekeeping, meal preparation and grocery shopping.
Qualifications & Experience
  • Certificate III or above in Aged Care or Individual Support (Ageing).
  • Open Australian Driver's Licence and your own insured, roadworthy vehicle.
  • Experience of 6 months or more in a similar role, with experience in personal care, manual handling, hoist training and medications.
  • Proof of vaccination against Diptheria, Tetanus, Pertussis (whooping cough), Hepatitis B, Measles, Mumps, Rubella (MMR), Varicella (chickenpox), Influenza and Covid-19.
