About the Role

As a Senior Service Delivery Coordinator (Disability), you will step into a key role responsible for overseeing complex participant supports, mentoring a team of support workers, and ensuring service delivery aligns with NDIS Practice Standards. You will provide operational oversight while working closely with participants, families, external stakeholders, and allied health professionals.

This role includes responsibility for creating service agreements, and developing and implementing support plans and reviews to ensure responsive service delivery that reflects NDIS Practice Standards, contemporary service principles, and exceptional care.

You will manage a medium-sized caseload, working closely with participants, families, support coordinators, and multidisciplinary teams to deliver person-centred, high-quality oversight of supports. Key Responsibilities

• Initiate and maintain productive relationships and strong rapport with participants, representatives, stakeholders, and service providers with differing priorities
• Develop, review, and update comprehensive case plans, risk assessments, and allied health reports
• Conduct regular medication chart reviews, manage medication apps, and oversee participant medical appointments and reviews
• Provide staff training, clinical competency assessments, and support worker supervision
• Investigate incidents, conduct clinical reviews, and maintain compliant documentation in line with NDIS standards
• Maintain and update internal records, care plans, and progress reports
• Oversee the quality and consistency of Just Living's support services to participants

Your Experience and Qualifications

• Demonstrated knowledge and understanding of NDIS compliance, practice standards, and industry processes
• Sound understanding of NDIA Quality & Safeguarding Commission reportable incidents and reporting requirements
• Experience in day-to-day management and oversight of quality service delivery in the disability/community services sector, including work with people with complex needs
• Strong clinical judgement, independent work ethic, and ability to work effectively within multidisciplinary teams
• Experience developing and overseeing care plans, participant support strategies, and providing training to support staff
• Current SA driver's licence, First Aid certificate, and required clearances
