Job Description:

Role title: Clinical Nurse Specialist (Grade 2)

Role type: Max term contract part time (16 hours per week until August 2026)

Department and location: Chronic and Complex Care, St Vincent's Hospital, Darlinghurst

Salary: $113,243.15 - $116,963.48 (pro-rata) + super.

About The Role

• The Chronic Health Nurse is responsible for navigating care of the patient and services that facilitate a person with a chronic disease to manage their health in a way that will result in optimal health outcome.
• A creative, flexible and adaptable person who is able to think outside the box and be good problem solver whilst managing a vulnerable and complex cohort of patients.
• An extended leadership role in the coordination of nursing and health care and passionate about quality improvement, whilst working within a multidisciplinary team.
• Providing care in a range of modes and settings including home visits, MDT clinics, virtual care and group classes.
• Collaboration with many external stakeholders including but not limited to Primary Health Network, NDIS, My Age Care, other Local Health Districts and Non-Government Organisations'.
  
  

About You

• Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
• Relevant post-registration qualifications and at least three years' experience working in the clinical area of their specified post-graduate qualification.
• Current unrestricted NSW Driver's License.
• Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
• Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse.
