About The Role
Western NSW Local Health District (WNSWLHD) is strengthening its model of care dedicated to women's health rights and self-determination. Our mission is to ensure all women in our district have access to the care that they seek, providing central point of contact for a comprehensive wrap-around service. Our goal is for consistent care, no matter the location and we are the first LHD across the state to implement such a service. We are currently building a team of passionate and motivated clinicians, technicians and support staff who share our mission to provide choice and improved health outcomes for the women of Western NSW.
The Social Worker role is suited to a clinician who wants to make a real difference to the access to care and support for women in WNSWLHD exploring or exercising their right to self-determined care. You will have a history or proven passion within the sexual/reproductive health space and preferably some experience in community care.
You will provide a high quality clinical service to the women of WNSWLHD through clinics and outreach services to the surrounding towns. As a complete wrap around service, you work with the team to facilitate supports for everything involved in accessing this service.
Rolling out a new model of care, you will be given a great deal of autonomy to advance and improve this comprehensive service in your area of expertise, and contributing to the team in quality improvement initiatives.
This role reports to the Child, Youth and Family Division, part of a tight team who collaborates well together. Your colleagues will include a clinical nurse specialist, midwife, an Aboriginal health practitioner, pharmacy technician and administration support. It is predominantly a Monday to Friday role, with regular hours and no shiftwork.
As the first LHD to implement this approach to care delivery you will need to be very well organised - keeping excellent records and high quality data to prove the service success. The role also requires some flexibility with clinicians transitioning into other related patient service within the divisions, if projected service usage is not met.
