About The Role

Western Health has an exciting opportunity for an experienced and motivated pharmacist to join our talented team as a Grade 2 Clinical Pharmacist – Aged Care. This position is permanent, full-time (80 hours per fortnight) and available from 01/07/2025.

Working under the guidance of both the Senior Pharmacist – Aged Care and Senior Pharmacist – Transitions of Care, you will be part of a growing and supportive team of skilled clinical pharmacists dedicated to delivering high-quality, patient-centred care across our aged care and transitions of care services.

This is an excellent opportunity for a pharmacist with demonstrated clinical expertise and a strong interest in aged care and transitions of care to expand their impact in a progressive hospital environment.

Key Responsibilities

Support the Senior Pharmacist - Aged Care and Senior Pharmacist - Transitions of Care in providing aged care and transitions of care pharmacy services across Western Health.
Actively participate in service development, including contributions to departmental projects, research activities, and quality improvement initiatives aimed at improved aged care and transitions of care services.
Work collaboratively across the multidisciplinary team to ensure the delivery of high-quality, patient-centred care within the aged care and transitions of care settings.

About You

To be successful in this role, you will:

Demonstrate at least 2 years of broad clinical pharmacy experience, ideally with exposure to aged care, general medicine, or transitions of care, and a strong commitment to high-quality, patient-centred care.
Apply expert professional knowledge to optimise patient outcomes through safe and effective use of medicines, particularly in vulnerable patient populations at transitional points of care.
Have experience in orientating, mentoring, and supervising clinical pharmacy staff, interns and students, and serving as a positive role model for junior team members.
Demonstrate enthusiasm in contributing to departmental educational and operational initiatives, including quality improvement activities, audits, and practice-based research.
Display excellent communication, interpersonal, and time management skills, with the ability to collaborate effectively within a multidisciplinary team.
