Experienced Social Worker required for locum in regional Victoria. Join a team of Allied Health Clinicians.

Responsibilities
  • Caseload coverage includes Transitional Care Program, Rehab in the Home, and GEM Program.
  • Collaborate with external providers and the My Aged Care system.
  • Liaise with family supports, explore long-term accommodation options, and address other social issues and general patient support and education.
  • Work with a multidisciplinary team; join one other senior Social Worker on the team and an allied health team of approximately 10 across these programs.
Requirements / Details
  • ASAP start for 8 weeks with possible extension.
  • Knowledge of Victorian and NSW Aged Care systems preferred.
  • A driving licence is required.
  • 5 days per week, Monday to Friday.
  • Free accommodation and one-way travel provided.
