About the role
The Exercise Physiologist forms part of the Wide Bay Community Resilience Program, a grant-funded program through the Country to Coast Primary Health Network (CCPHN). In this community-based role, the Exercise Physiologist provides comprehensive assessment of vulnerable older-adults physical, environmental, and social circumstances, exercise program prescription, and supports referrals for ongoing care. This role reports to the Manager Clinical Services.
Knowledge and Skills
- Assess vulnerable older-adults physical, environmental, and social circumstances in the community, with respect to disaster preparedness
- Conduct accurate physical and exercise capacity assessments regarding older adults with chronic health conditions
- Support clients by identifying and actioning any of three referral streams available for ongoing Specialist Support Worker assistance (Exercise Programming, Environmental Management, and/or Social Connection), where required.
- Liaise with Intake and Referral Officer to optimise timely referral management, client journey and discharge, and facilitation of program data collection and reporting
- Liaise with client stakeholders and referrers to provide a high-quality level of multidisciplinary care
- Provide support and mentorship to Specialist Support Workers to deliver prescribed exercise programs
- Promote and present to relevant partners regarding program awareness, scope, and outcomes (PHN, GP practices, QAS, etc.)
- Engage and develop relationships with relevant volunteer organisations
- Contribute towards developing a feasibility study to highlight program effectiveness
- Schedule and cluster appointment times efficiently across the Bundaberg and Fraser Coast regions
- Collaborate with the Community Resilience Program team to deliver meaningful outcomes for clients
- Prescribe clinical exercise programs in the context of increasing client?s functional capacity to manage in disaster preparedness
- Ensure accurate and timely record keeping required for program reporting purposes
- Participate when required in internal and external audit and quality assurance activities, with respect to continuous improvement of exercise physiology service provision
- Under direction of ACS leadership team, participate in project or program development activities to ensure contract deliverables are met
- Experience working with older adult clients 65 years and older
- Experience with communities with low health literacy and socioeconomic disadvantage, and mental health and wellbeing challenges
- Demonstrated experience in a multidisciplinary care environment
- Experience with development, implementation, and evaluation of group programs
