Journey Coordinator – Stepped Care
4 days ago
- Are you interested in being part of one of Australia's leading regional and community-based Mental Health Service providers?
- We have Part Time and Full Time roles available
- Excellent starting salaries $73,000 - $85,000 (commensurate with experience), plus 5 weeks annual leave.
Come for the life experience, stay for the lifestyle
North and West Remote Health (NWRH) is currently seeking adaptable Journey Coordinators within the Stepped Care service system to join our team.
The Journey Coordinators are dedicated positions within the Stepped Care service system that operate as an independent partner in a person's care journey. They may be a standalone service, delivered within a Front Door service, or delivered as an adjunct to another Stepped Care service offering. The Journey Coordinators will ensure that individuals are placed at the centre of their own health and wellbeing, and experience improved care and mental health.
Your primary responsibility will be to deliver high-quality support and services by improving outcomes for Stepped Care participants. You will continue to build psycho-social education, support access to self-management approaches including digital mental health services, and facilitate the Stepped Care model to ensure the quality, continuity, and coordination of care between service providers.
This role is offered on a full-time or part-time basis to suit your lifestyle. This role is based in Townsville and has outreach travel.
It's challenging, but we make a difference.
NWRH is seeking dedicated individuals who are culturally sensitive and adaptable, with the ability to work both independently and autonomously.
You will have high-level communication and negotiation skills to establish and support respectful and trusting relationships with individuals, teams, NWRH staff, and the community at large.
Our teams come from across Australia and include a wide range of ages, skills, and specialisations, so you'll be working with the best This variety enables us to collaborate in our engagement with clients and learn from one another's viewpoints.
What is Stepped Care?
The new stepped care model has five principles:
- Support starts at the time of asking
- Every interaction builds confidence in the system
- Overwhelmingly compassionate
- Outcomes that really matter
- The system stretches, not the person
These principles provide the foundation for the future system and will guide service delivery.
About you
We are looking for individuals who have the following qualities and experience:
- Ability to establish, develop, and maintain multiple productive working relationships and networks
- Experience identifying and delivering partnership and collaboration opportunities that enhance consumer outcomes and improve the service system
- An understanding of mental health challenges and how they inform an individual's experience
- Ability to develop and maintain a genuine relationship with individuals accessing Stepped Care
- Ability to adopt a recovery-focused, person-centered, strengths-based, and trauma-informed approach
- At minimum a Certificate IV in any Mental Health-related qualifications. Only those with suitable qualifications will be considered.
If you have a solid sense of connecting with communities and are genuinely interested in making a real difference, this could be the role for you
You'll be joining a fantastic support network, from your colleagues across QLD to the wider NWRH team of mental health professionals and allied health professionals.
In return for your hard work and dedication, you'll receive an excellent starting salary, commensurate with your experience, PLUS a range of additional benefits, including:
- Up to $15,900 salary sacrificing;
- Annual leave loading of 17.5%; and
- 5 weeks annual leave;
- Relocation assistance;
- 10 Days Paid Professional Development Leave;
- Continuous Professional Development;
- Paid Parental Leave and Paid Partner Leave;
- EAP for you and your family;
- External supervision benefits;
- Work with a supportive, dynamic, and innovative leadership team;
