About the role

Join a supportive, multidisciplinary team delivering OT services to older adults in the community. This role offers full flexibility across Sydney metro, with work tailored to your home location and no office-based requirement. You'll manage a caseload of Home Care Package clients aged 75+, conducting in-home assessments and functional reviews.

  • Manage aged care clients (75+) in their homes across Sydney
  • Focus on home mods, assessments, recommendations, reviews
  • Fully work-from-home model - no clinic/office attendance required
Skills & experience
  • 2-3 years' experience as an Occupational Therapist
  • Background in community care or aged care preferred
  • AHPRA registration (unrestricted)
  • Strong clinical reasoning, independence, and time management skills
Benefits
  • Up to $108,000 p.a. + super
  • Car allowance for car hire or fuel allowance if using your own car
  • Additional $50/hour for hours beyond 30 billable/week

The Role

As a therapist at Connect2Care, you will be joining a multidisciplinary team. You will provide evidence-based services to people through multiple service delivery models, including telehealth, community visits and centre-based services. Referrals may come through the NDIS, Medicare or privately. It is a mobile role, with the option to work from home or our hubs when not seeing clients. The role will involve the following:

Assessing and providing interventions for children & adults living with a range of disabilitiesWorking within the NDIS to maximise clients' level of function, capacity & social engagementAssisting clients with assistive technology assessment, recommendations and applications (in relevant fields)Timely documentation & report writingManaging a varied caseloadMeeting company professional expectations (e.g. meeting billables & effective client management) Attending meetings, supervision & personal development (as required)Building relationships with key stakeholders, including therapists, clients, & referral sourcesWorking across leading software platforms & online tools
