About the role
Seeking a Level 3 Social Worker to provide psychosocial assessments and complex discharge planning for two inpatient oncology wards with a large elderly patient population. Collaborate closely with medical teams to deliver patient-centered care in a supportive, part-time role.
Key responsibilities
- Deliver psychosocial assessments for oncology inpatients
- Lead complex discharge planning with multidisciplinary teams
- Provide support to patients, families, and carers
Skills & experience
- Degree in Social Work
- Current or eligible AASW registration
- Strong skills in discharge planning and assessments
- Fully vaccinated for COVID-19 (including booster)
